Once you have made the decision to enroll in the DTCM/DAc Bridge Program you are ready to begin the application process. To be considered for admission to Five Branches University, the following items are required:

Digital photograph, passport style and size
A non-refundable $65 application fee payable by check or money order to Five Branches University. Please indicate which program you are applying to.
An up-to-date resume listing all previous education, job experience, accomplishments and volunteer work
(if applicable)
From all colleges and/or universities attended, including Master’s and graduate level coursework. Academic transcripts must be emailed or mailed directly from the academic institution to the attention of the Santa Cruz admissions office.
